I know it’s meant to signify eating out a girl. Is it just like an thing you do around “the boys?” I’ve seen so many of my guy friends do it even in their 20s.

I can't speak for men, but I only use this gesture to tell my boyfriend he should come to our bed and eat me out.
If he wants that too, he'll make the same gesture and usually follow me immediately, or he'll decline the offer.
It's an easy way for me to initiate sex in my relationship.

It's a playful gesture. Some do it to have fun and not mean anything by it. Some use it as a way to non-verbally cat call at a girl. Some use it to show what they would do to you and use it as a non-verbal way of asking.
