it's possible that your metabolism is not accustomed to building muscles and other tissues but is more catabolic or tending to break what macromolecules you ingest and therefore you do not absorb enough nutrient value. it might be genetic but still seek out an MD for advice as soon as you can to help you get a proper diet and to pinpoint your problem.

gee you sound like my husband, he is trying to gain weight and he says he eats a lot but he just can't gain any weight at all.
you can google how to gain weight safely and there will be some helpful hints if you like.
I tried that for my husband. I think it's just about eating more, and eating more does not mean eat unhealthy

My brother had this problem... your may be completely different, but do you ever have trouble breathing, asthma or allergic reactions that cause breathing troubles? My brother was diagnosed with Cystic Fibrosis, he's 26, usually diagnosed in child hood but some people only shoe bad symptoms later in life.
Get checked for parasites.
