It looks like it, i mean his skeleton and well everything is expanding dramatically at an outstanding rate! And turning back is just has painful, on a side note what happens to all that extra mass such as cells come from/go?

It varies. Some cases it's painful, some cases he's immune to the pain from how much rage is on his mind.
The Hulk also is believed to always be in some pain? Thanks to the radiation but I don't know if they make much awareness to that and how that differs to she hulk or even with how smart hulk acts so casually and not angry or in pain.

I've had some discomfort before... when wearing something TOO fit, but I would not say it was painful

I think it even explicitly says somewhere in the Hulk comics that the Hulk finds his transformation painful.

It probably does. But at that moment the change is already taking place. So I think the question is does the incredible hulk feel pain? How much? And if it's minimal then if Bruce is feeling pain mid-change. It's probably very temporay

I don't seem to feel pain when I'm angry. Something to do with adrenalin. Unless it's the pain that made me angry in the first place.
