Here's the way I look at it.
If it's a "playful" take on your name. It's a friendship thing. Example I work with a woman who has the last name of Di Iorio (pronounced Dee-ore-e-oh) I nicknamed here "Oreo" like the cookie.
If it's a compliment on one of your better characteristics, Physically, personality, emotionally, compassion etc. There are thoughts of more than a friendship and he's "testing the waters" to see what kind of reaction he gets. Positive or negative.
If is something like "buck toothed horse face" you might want to consider that being mean. It's not something you would like to have anyway. Unless of course you think it falls under the second category. Then... JK ;-)

It depends. I gave this girl a nice nickname that I used for a couple of months. She is a special lady but the nickname was harmless so it wasn't "obvious" that she was special. After we got into a fight and I tried to apologize, she wouldn't let me use the nickname again, which meant that it meant something to her, because at first she told me she didn't mind if I called her that but after that fight, she told me to stop. So, in my case, if I give a girl a nickname its because she is special.
It was my first and only time giving one. We are talking again now, but I haven't used it.

I don't think it usually means much. I suppose it might depend on what the nickname is. My friends and I often give nicknames to people and don't much care about it.
Nicknames are just a fun or mean thing men do. Nothing much else to it.
