+1 yI have some issues with the nature of government and society as we know it, but those are more complicated to argue and I just don’t have the energy today, frankly, haha.
A simpler one is The Industrial Revolution. The human population has become unsustainable and is only exponentially multiplying, and I’d argue most of it is as a result of the over-survival of our species due to comforts provided to as a direct or collateral result of the unnatural circumstances The Industrial Revolution has allowed us to create for ourselves. No other species does this, and I don’t take our ability to think of or tangibly produce these results as some automatic seal of approval for doing so. Just because you CAN is not license to justify that you SHOULD. And at the end of the day, the goal of advancing the human species is only a concern of the human species, not the goal of the governing system of nature our species and all other species reside in, it’s only a self-centered effort for our own comfort, whether physical, or emotional, in our avoidance of negative-but-natural outcomes in life.

Some of the biggest mistakes from the perspective of those who made them include:
a) The destruction of much of the Roman Army by Germanic barbarians in the Teutorburgerwald in AD 9 during the reign of Octavian.
b) The Byzantines trusting the Venetians in 1204 during the Fourth Crusade, which resulted in the sacking, occupation, and destruction of their capital. It wasn't recovered until 1261, and the empire never recovered from that disaster, and that in turn directly led to the eventual fall of the empire to the Ottomans in 1453.
c) The Manhattan Project during World War 2. I would rank this as being THE biggest mistake we've ever made, because it will almost certainly be the ultimate cause of our extinction.
d) Napoleon invading Russia, Hitler invading the USSR. We all have Russia to thank for successfully resisting these murderous tyrants. The world of today would be a VERY different place if they had won.

+1 yHow do you quantify "biggest"? If Custer had just assigned better horses to tow his Gatling guns, Little Bighorn wouldn't even have been a battle; just a slaughter. It got him and all his men killed-but in the grand scale of civilization, it's not that big. The Japanese government saw the US as decadent and rich (which it totally was) and assumed that meant it was weak, ignoring that the US got to BE decadent and rich because it was strong (they also thought it was corrupted by interracial breeding and Jitterbug dancing; clearly, they weren't paying that much attention). A much bigger mistake- but look what happened to Japan over it. Yeah, all their cities were flattened, but today, they're a thriving, free nation, with exemplary dedication to human rights and one of the major economic powers of the world.

Anonymous(36-45)+1 yThe biggest mistake is to think that progress is linear, to abandon old values assuming them to be out-dated/no longer necessary. This usually happens once a civilisation reaches it’s peak of success, but it only reached that level of success because the civilisation was strong as a result of those values.
Once this happens it’s only a matter of time before that civilisation falls either because it will be invaded or will collapse as a result of internal conflict.
