A smoker yes depends on the drinking part though. One or two or even three drinks is ok more than that for me a turn off.

Controlled drinking no.
If you take alcohol but don't get drunk at all then I have no problems with that.

Drinking occasionally is cool. Any kind of smoking is not okay with me and I probably won't wanna date the person.

Yes, both are a turn off since it means they aren't taking care of themselves. It's fine if they drink, but not to the point where they can barely stand up.

Anything done excessively is a turn off. Not a huge fan of smoking but its not a deal breaker.
