I think it's because Alluka was originally born male since her brothers (except for Killua) refer to her as a he. But Killua calls her his sister and corrects them on her being a girl.

I've heard a lot of people say it was a translation error but personally I kind of like that it layers in more nuance. Her older brother and paternal relatives tend to mis gender her but Killua doesn't, it really fits with the close relationship between the two.
