I would tell them to give me proof, either with a doctor's statement or a health organization's confirmation that this person is in his/her last stage of life and that this person is too weak to write their last words or to dictate them.
There are so many weirdos in this world that I would have to ensure that I am not the victim of a prank and that this person is only out there to make fun of those willing to help them.
Just holding their leg in pain is not proof enough for me that this person is about to depart.

It's okay. I would probably let the dying person speak into the microphone of a smartphone. That way, the message is aural and has a better impact for the recipient of the message than any written note. The dying person may be too weak to write and me taking notes of his last words is not the same since it is not the person's own handwriting.

Can I not just record them on my phone? Or take a video so the true meaning can be seen and heard
Depending on the circumstances. I would would do my best to fulfill their wishes.
