I just feel like by saying "I'm lactose intolerant" you're kinda saying "I'm gonna be shitting my brains out if we go get ice cream" so I'm wondering whether other people get self-conscious about this haha

To me, speaking up about your wants, needs, limits, and boundaries is a must. If you can't tell me you're lactose intolerant, how the fuck else will you handle other, much more important and embarassing conversations in the future? Avoidance?
No thanks. I'd rather not be caught surprised by whatever diarreah that fallout causes.

I'm not lactose intolerant but I would just tell them that I was so there would be no problem. I'd probably be self conscious about it because most people know what that means if the person has dairy. But hey life is life and none of us are perfect. The right person won't give a shit no pun intended. They accept you for who you are.
If you don't want to say it directly, just get a lactose free option.
Pop several lactase pills before eating lactose. Problem solved.
