A female customer at my work said "I didn't know you two you were... an item?" when she saw my boss and I talking, she seemed very curious about it.

It certainly can mean that. But... not in every situation. This could just be nosey woman being nosey. People are curious about that sorta thing (well a certain type if anyway). You need to look at the context. If she was flirting with you in other ways, then "yes" that's what it means. If she was just curious... probably a "no"

She comes by a lot and we usually talk about how things are going, and I can usually make her laugh.

Then It could indeed be her reacting to the relationship with disappointed surprise (because she was/is interested in you). It depends on your read of it because you were there to see all the body language, tone of voice etc. It could still certainly be simple curiosity. But it definitely could be interest. I'm leaning towards interest if she seemed flirty (or interested) during your previous interactions.

You're going to have to determine which it was yourself. It's a "you had to be there" problem. Only can know which is more likely

It means nothing, usually. Just making small-talk. Unfortunately, insecure, immature men will think it is flirting or something.
