Anyone know how to fill out disability application?

OK before you judge me I am not a lazy lowlife trying to get money off my kid for free.The run down is this I have 4 kids ages 12-5 and Their father is in prison for 20 years due to beating me and other horrible things. I have no family at all to help me.My father molested me as a child and my mother ran off when I was 10. Everyone else either dead drugging or I don't know them.My 10 year old son is having really bad behavior issue because of his father. I have been struggling trying to keep my job and my sanity with my child for 5 years now. Last week he tried to burn my house down twice.I admitted him into a behavioral health hospital. It is in another state that takes 30 min to get to every 3 days and not to mention all the other places I have to go for parenting classes to learn how do deal with the diagnosis's he was given this week. He has adhd, odd, add, and ptsd. I need extra money for gas and counseling and other things related to this stuff going on with my son going to his school once a week to get work to take to the hospital.I went to ssa.cov and it says am I applying for myself or applying for someone that is with me or applying for someone who is not with me.My question may sound dumb but to me there is no dumb question. ANYWAY should I chose applying for myself since he is my minor child or chose applying for someone who is not with me right now because he is in hospital? I do work full time but it's just me and 4 kids on 9.00 hr. Can't get child support because dad is in prison and welfare says I make to much!

If you have a disability you are entitled to disability, the thing with disability is you have to be proven to have a disability like you have to have doctors forms etc but you can try, just print out an app online then you will be asked to do a phone interview

Of course you're absolutely right on this. If you're dealt with your doc for a while then it should be no problem getting him/her to verify your disability

if you in texas the form is H1836-A which has to be filled by a DOCTOR

you can't fill it ,a doctor has to do it

im sure its the same in the other states
