No underwear at doctors appt?

I've got a physical exam coming up and then a dermatology full body skin exam appointment a week after. Not sure what to expect during either, as I haven't had appts like this before. I normally free ball, and expect them to say I need to disrobe at some point.
What should I expect during these exams? Would the doctors think it's weird if I normally don't wear underwear?

Any interesting experiences that you've had in similar exams, please share!

For what it's worth, they are both female doctors and I'm a decently in shape guy.

Doctors and nurses really don't give a fck, what you wear!! They have seen everything, and if you are getting naked, they really don't care!
I can't imagine what their personal sex life is like, seeing so many people, of various types, all day, then coming home, and. . .

They will likely have you go into an exam room, and have you disrobe with no one else in the room and put on a gown that opens in the back. They probably won't even know that you were free balling.
