I know that here in the states you're safe with your doctor or at least supposed to be.
My Mom took me to get a full exam with the purpose being to catching me having marijuana in my system. The doctor tried to talk me into telling her but he asked me and I was like "no" and as my Mom asked the doctor directly about marijuana use, he lied for me.
It didn't get my Mom off my back by the way, she just started looking for other reasons to hammer me for being something fucked up. If someone has it out for you, they'll either find something or make it up eventually.

If it were in the UK then i would say no as you are no longer under parental stuff, i suspect it will come up itemised as "something" which he can query but will probably be told they can't tell him, at which point he will come to you, thats my best gues

If you are in the US, yes he can see the explanation of benefits statement that either comes in the mail or is emailed to the plan sponsor, which is your Father.

You don't say which country, but you're an adult, so you're entitled to confidentiality. I doubt your parents are sent an itemised list of doctor's visits.
