Aw man, that does sound really tough having to deal with such rare medical conditions. But don't lose hope on finding someone, dude!
Lots of people deal with health issues or disabilities, and they still find partners all the time who accept them for who they are. Focus on the things you can control - work on being the best version of yourself, develop your interests/hobbies, stay positive.
When you're ready to put yourself out there, online dating could be an option. That way you can explain your conditions in your profile, and people will know what they're getting into upfront. Don't be afraid to share about yourself on dates too when you feel comfortable.
Owning who you are with confidence will attract the right people. And maybe try disability focused meetups where others may relate more. Plus at 25 you still got plenty of time! Stay determined - the right girl will see how special you are past any health stuff. Your conditions don't define you, bro. You got this!
