Is there a reason certain women still refuse an epidural?

I'm currently pregnant for the first time and definitely getting an epidural. I want to have the most comfortable childbirth experience with the least amount of pain possible. I hope it's not like what I've always seen in movies as a kid.

I can already imagine the overwhelming pain during childbirth before those injections were invented. Even though I'm health without any pre-existing conditions, what if I faint as a reason of too much pain? Then again, it's my first time.

It's considered a reliable, safe procedure and takes away most of the pain so why do certain women still refuse an epidural?
