Eat a calorie surplus (approximately 300-500 calories per day more than your DRI).
Make sure your diet consists of healthy fats, lots of protein and good carbohydrate sources.
Adopt a workout plan that revolves around both resistance (weight) training and cardio. The cardio should be moderate intensity (brisk walk or light jog), approximately 20 minutes a day on weight days and 40 minutes a day on non-weight days. Weight training should be 3 days a week to start. if your goal is muscle building you should do sets of high weight for lower reps and more sets.

Make sure you have enough electrolytes! You can get those from Powerade or Gatorade or even Pedialyte. That should help you feel less tired. And protein is your best bet! If you work out after having protein you can build muscle and it will help you gain weight. you'll want to do some weights like leg press and stuff. And ab work. also try increasing the amount of calories you consume in a day. But you should go see your doctor to ask for professional medical advice about this, because weightloss and weightgain can get risky.
