Your poll is inadequate. Genetics causes PREDISPOSITIONS to disease, higher risk. It's no guarantee, and the genetics don't CAUSE the disease. But between two people with similar diets and activity levels, the one with the bad genetics is more likely to develop the disease.
The question is kind of pointless... There's no opinion to be had here. This isn't some esoteric, rare disease. Genetic risk is a well documented element of heart disease.

I would venture to guess it's a combination of factors but having family traits of heart disease most certainly increases one's chance to also have it.
It's partially hereditary. Whether you get it or not also depends on your diet, how much you exercise etc. It depends on how well you take care of your body, as well as your genes.
Technically, yes, I guess you could be very healthy and still get a heart disease if your family has a long history of heart diseases. But then again, the risks will lower the better you take care of yourself.
Its partially hereditary as everyone in my family is prone to high cholesterol but diet has a lot more to do with it. If you eat really shitty you are going to get diabetes and die. Also all of the chefs in my school have health problems and some have had heart attacks because of their terrible eating habits

It might be a combination - inherited through genes but also through bad environmental conditions or choices.
