Sometimes, people get into habits that are hard to break, even though they might've "stopped" years, or even decades ago. It's sort of like one accidentally shows up because it was the norm years ago and you've become so familiar with your new love, your verbal guard drops and you accidentally call her by a name you used to use on someone else years ago. Or, It's like a nick name. You get used to referring to someone by that name so, you just automatically go back to that when you see them, again. You're not being disloyal or having thoughts of going back to your ex. It's just a slip of the tongue. I'd be willing to bet everyone does it at least once.
My grandmother had 9 kids (but only 6 made it to adulthood) and, very often, she'd be talking to my dad, whom she was living with, at the time, and would go to mention him by name but had to start the names of all of her kids before she'd eventually get to his name. "Well, Edwi, ah, Chu, ah, How, ah, Wal, ah, Shir, ah, Robert...". Even though she rarely ever saw any of the others except Edwin, who lived a block away, she still couldn't remember that she was talking to dad, who she saw all day, every day.

No, That would have gotten me so angry lol. I'm not the jealous type but when I was pregnant all of my emotions got exaggerated. I don't refer to any of my ex's as pet names and neither does my husband.
