I've heard people say it just to be nice. My personality though, I mentally add it to my to do list and work to make it happen.

Yep, exactly. If you want it to happen, you have to make it happen. I've moved around a lot, and lost many friends and acquaintances because we didn't put in the effort to keep in touch.
Instead of second guessing that they meant, decide for yourself if you want to keep in touch with the person. If the answer is yes, then make it happen. If they blow you off, then you'll now the answer. It's also possible that they give another ambiguous reply, so you still don't know. That's how it goes. It won't be the last time.
I would never say it if I don't really mean it. There's always a chance another person might take it seriously and then I'd really have to go grab lunch with them.
If I say it I normally mean it. But I see all of my friends every day at school so I don’t often have to deal with it.
I wouldn't say let's grab lunch sometime if I'm not planning to grab any lunch anytime soon.
