It's usually down to lack of education (family can't afford school or generationally children have had to work from a young age to support the home), and lack of access to contraceptives.

It's a real problem, especially in the US.

And it's proven that increasing women's access to education, generally increasing acesss to contraceptives and teaching populations on their use, and investing in health care all lead to lower teenage pregnancy ratings.

It's more a societal and cultural problem, than just an individual one.
