Spots and 'angry' skin are caused by blocked pores. Do you find that our have really oily skin or hair? It doesn't have to be everyday, but just sometimes? This could be because your body is producing to much sebum which blocks pores and causes nasty spots and skin irritation.
To reduce the production of sebum, I recommend introducing more Zinc, Vitamin A (I drink a glass of pure carrot juice everyday, the vitamin A also helps with skin tone) and omega-3. You can get these as vitamin supplements or in healthy food. Because these are all natural ways to help your skin they don't cause further skin irritation like facial moisturisers and cleaners would.
Spots are also a result of sometimes an over active hormone or a hormone that's not active enough, you can get this tested at your doctors. Sometimes its good to see a dermatologist, this will inform you if there is a chemical/substance in any of the products you use that's causes your skin to break out all over.
Also, regular exercise makes a huge difference because it helps control your skin glands.

It may not be the best but it is very effective. I know you probably won't believe me because it seems counter productive. I use coconut oil. The stuff you buy in the grocery store. It lasts a long time so it really is not expensive at all and a little goes a long way.
It absorbs really quickly so it does not sit on your skin, clogging the pores and there is something in it that helps kill acne bacteria.

If you want to go the medication route, I would go with accutane. I did it over last summer and the results were DEFINITELY worth it. I used to have pretty bad cystic acne on my face, chest, and back. Since doing accutane though, haven't had a single cystic pimple (its been about a year) I still get the occasional pimple, but nothing crazy. I know accutane does get some negative buzz surrounding it but for me personally, I had zero negative side effects. In fact the only side effects I had were chapped lips and dry skin. But, I would advise you to do some research before giving it a shot as it effects everyone differently.
I gotta agree with T-Time19, Accutane is half of the solution. The other half is eating healthy.

Go to a dermatologist. I had really bad acne that I took Accutane to get rid of and now I rarely get pimples. The acne scars are still a problem, but those are fading now too. I tried all the store bought stuff and even Proactive, none of it worked on the type of acne I had (which sounds similar to yours).
I had severe acne once upon a time. I personally have found that natural remedies are best. A green tea wash every night, nearly got rid of it all, within a month or so.
