If he's been unfair and unfaithful to you twice, what makes you think that he won't do it again?
If I was him I would've not asked for the third chance. I would've broken up and would've found new partner with whom I more compatible with.

It seems like he didn’t care about me. On my birthday he promised me he’d send a present box to me soon. (We’re LDR) but then he made me wait for 6 months already. He said he’s been busy but he went out so many places. He even was on vacation 2 times already, a week for each. I’m not materialistic but I just think he doesn’t care about me at all. I already told him about that. I gave him the second chance. He said he’d be better. But he still did nothing better! I wanted to break up but I still really love him. So now he’s asking me if I don’t wanna give the third chance. What should I do?

Oh my god. Honey. Why is he "asking you for another chance". The way to remedy this is to get you the most amazing gift box of the century, immediately! That takes like half an hour. Think about it. If he was willing to do what it takes to ensure you know he thinks you're special, he could have done it in half an hour. Instead, he wastes your time making you anxious, giving empty promises and heartache. This guy is a deadbeat. You deserve to feel safe and treasured. This guy cannot give you that.
