I asked him how he feels about being exclusive and he says we are exclusive after telling me he isn’t seeing anyone else. Does that mean we’re in a relationship or should I ask him if he wants to be my boyfriend to be sure?

What does "girlfriend" mean to you? To me it means spending a lot of time together, being generally willing to help if the other needs assistance, basically behaving as a couple without living together. If a couple of living together then most people use "partner".

The fact he said exclusive implies it without actually saying it but I mean there is no harm is asking him
