I dated a guy with no nipples, so i am sure i have no problem with that, as long as not like huge black spots on his face.

Damn, that sucks. Well, I guess makeup is your best option.
Think about this. A tattoo. I once dated a woman briefly who was a tattoo artist specializing in reconstructive surgery patients. So, women who had breast reconstruction after cancer, etc. She showed me client photos of before and after where she had created areola's and nipples for clients. It looked pretty damned good in photos. She also worked with hiding scar tissue.
Give it a thought and maybe try and contact a reconstructive tattoo specialist. Sort of permanent makeup? It may not work 100% but say it covers it 80%, that could be a win, right?
Yeah, makeup works in this case.
