You cannot be denied time off for a medical appointments or procedures:
"I can't make it in. I'm on my way to the hospital for emergency surgery to have my appendix removed."
"No way, we need you here today... and be on time!"
As @btbc92 said, you'll need a doctor's note to back it up, and don't be surprised if a future non emergency time off request is denied out of hand (employers have ways of getting even).
Whatever the reason for the hospital appointment, I hope it's nothing serious.

No your Boss should be understanding. I would contact him as soon as possible thought so he has time to cover your work.
Just tell them this. They have to find a way to handle it, and as long as you have a doctor's note to show and prove that, they cannot say much of anything. However, it may mean you cannot take any more days off in the future, unless a serious emergency. Use your off days wisely because you may not be able to get vocations days also. Check your work policies especially if you new to the job.
