Should I report this teacher or do you think it'll back fire?

Today she stated clearly that she might have to fail me. Also this teacher isolates me in a group of four students, and tells me I can't give meds or I can't have two patients, and she says this infront of the other students. She also favourites this one girl and everything she does is okay and if me or someone else does it, its not. I have two more classes and I feel very down because of the stress and the way she's treating me. She also always use my mistakes as bad examples every class, and her favourite student always has the best examples even if she messes up. This has happened to another girl before too. This teacher bullies and student and try to isolate them. I feel like the other students are starting to ignore me and single me out because of the way she treats me and they all fear them.

Assuming you are doing everything you need to do in the class and aren't just being spoiled or entitled... You should tell the faculty about the nursing teacher and how the teacher is being unfair to you, but tell them not to interfere until after the class is finished, to see if the teacher actually fails you or not. If she does, you have evidence prior to the fact. And if she doesn't fail you, no harm, no foul.

So warn the proper people your nursing teacher might fail you or is setting you up for failure, but ask them to keep it secret and not interfere until the end. And IF you do fail, then you'll have evidence she was going to do so, before she actually did it. Trying to do so after you fail won't do much to help you.

the instructor gave me a memo of concern, so i might get removed from the program. She made me miserable so I obviously made mistakes

I think you are talking about Nursing School? Anyway, it is a challenging course and I understand you are having an issue with one of your instructors. It could be intentional or maybe the instructor is not even aware of it. Instructors often have favorite students, I try to be as fair as possible with my students but I often have students that are just a better fit for my style of teaching. Maybe find out the instructor;s office hours, determine what parts of the lesson is a problem for you and go for extra help. The instructor will see you are willing to put in the effort and you will get a chance for the instructor to know you a little bit better.

You cannot run away from this. You need to talk to this instructor and see what you can do to improve. I have been there and it is what I did. I am not the smartest guy around and still I managed to graduate. I did it by working hard, seeing instructors when I had problems and in one course I actually hired a private tutor. You got this. Just keep pushing.

Is there any way to shift classes, or withdraw for this term and come back with a different instructor the next?

No, it'll be counted as a fail. Unless I report it :( I thought I could stick through it but it's not posssible.

It seems to me the only way out is through, if it will be reported as a fail at this point. Or as Winston Churchill said, "If you are going through hell, keep going!"

Yeah i know what you mean, it just feels like she doesn't like me and the way she treats me it makes me feel depressed and unmotivated and discouraged.

You could try, but if she has tenure, and odds are that she does if she has been there for years, it would likely make no difference. I think you just have to hunker down and get through it. If you have to retake it, does someone else teach it?
