Employee who won't do any work unless micromanaged, is forgetful, makes mistakes with everything, stares into space and says "I don't know" to everything when asked
I’ve given her resources for memory aid, provided graphs for processes, we have catch up meetings every Monday and Thursday, i’ve taken on some of her workload, i write steps down for everything i give her to do and suggested seeking medical help, which she declined to do. I don’t know what more i can do to help, it’s affecting everyone in the workplace and i’m now receiving complaints about her.

If he doesn't know I think you can speak privately to your boss. Instead of telling him that she's "messing things up", tell him that you're doing everything you can to assist this woman in her work but X happened nonetheless. You have to remember it is not your responsibility to fix people at work. You just do your job well and if all else fails, I'd suggest finding a new job because this doesn't seem like a healthy workplace.
Are you this person’s supervisor? I’m only asking because of how you described the situation. Anyway, are you aware of any cognitive problems?
