It's understandable to feel concerned about how your neighbors may perceive you when you experience PTSD-related outbursts. However, it's ultimately up to you to decide whether or not to address the issue with them.
If you feel comfortable doing so, you could consider speaking with your neighbors and explaining that you have PTSD and may experience outbursts from time to time. You could let them know that you're working on managing your symptoms and that you apologize if your behavior has been disruptive or concerning.
However, it's important to remember that you are not obligated to disclose your medical or mental health information to others if you don't feel comfortable doing so. If you prefer to keep your condition private, you could try to find ways to manage your outbursts in a way that minimizes disruption to your neighbors. For example, you could try using headphones or white noise machines to block out sound, or you could try to schedule your activities around times when your neighbors are less likely to be home.
Ultimately, it's important to prioritize your own well-being and to find ways to manage your symptoms in a way that works for you. If you're struggling to cope with PTSD-related symptoms, consider seeking the support of a therapist or counselor who can offer guidance and support.

I suggest seeking help. As to your neighbors don't worry about it. You'd have to have the windows open or be as load as a forklift to get through two houses walls and the space between them.
