Why would a guy try speaking to me around others but avoid me when we’re close?

There’s a guy I really like We’ve only had one convo 5 months ago.

Recently on shift a male patient was being inappropriate towards me and my crush asked someone else what happened. Then when I walked past him he made eye contact and said if anything else happens let me know and asked if I was okay.

Not sure why he was so observant as he was busy with some other task in another corridor but he had his eyes on me. He also could have just asked me directly? I also realised he wouldn’t come in corridors when it was just me and him and he waited in till I had gone. Just some awkward tension?

Awkward tension about what? It's the second time you've spoke to him five months... I think you are seeing things a certain way. (ie. projecting). He was doing what a normal coworker would do (regarding the incident with the male patient) What do you mean he could have asked directly? I don't get why that matters.
Don't try to assign any significance to any of this. If you are interested then talk to him.

I think he knows I’m crushing on him through someone else. We’ve also seen each other around in those 5 months. So when he saw me on shift he was surprised to see me after so long. And I mean he didn’t need to get involved because there was already other professionals to sort the situation out. Also he came out when I had my break and he made an excuse to leave and was observing me from far.
