What mental illness could this possibly be?

A buddy of mine behaves very strangely... first he's all nice and cheerful and he complimets me, then the following day he appears to be moody and say hurtful things, he's also very restless to the extreme - he can't even sit still for a 1 hour movie... he also bluffs a lot and makes up stories...

Could he possibly be bi-ploar? Or is he a compulsive liar?

I've also noticed that his parents are overly-protective of him and he's a grown man 23 years old. He's also scared of taking the bus - prefers to be driven or to take taxi cabs... he also makes fun of people and acts very silly.

He also giggles and gets overly shy when anything sexual is seen or heard on TV or by me or anyone.

His clothes also have small holes as well I've noticed, and he repeats himself as well at times

Maybe he has bi polar tendencies but it sounds more like he had a mood disorder. My ex had the same thing - very sweet and into the relationship one week, aloof and distant the next, sometimes cruel. He was also sexually shy and repressed. People that knew him described him as strange, odd. He broke up with me out of the blue and very cruelly. For a long time I thought it was me and my self esteem was damaged, but when I realized there was something wrong with him, it was such a relief.

That's not bipolar disorder. Bipolar disorder consists of highs and lows, that each last from days to months. Its not happy one day sad the next. Its more like super high for two weeks, very depressed for a few weeks.

And it sounds like he's got a personality disorder but no one here is a psychiatrist.

Based purely on what you have reported here, the most likely diagnosis is a pervasive developmental disorder.

Bipolar usually has longer cycles of mania and depression.

Don't say that. Bipolsr disorder is NOT what most people think. Its not just mood swings and anger. Bipolar moodswings have days, weeks or months between them.

He would be manic for at least a few days, then depressed a few more.

I was diagnosed with BPD 30 years ago (manic depression at that time). The mood swings can have days, weeks, or months between them. But not always. I don't know if he is or not. But if he is not on any medication his swings can be rapid. Even if he is on medication, and it needs increasing the dosage or maybe changed altogether. It is not working effectively and again you can be up and down like a rollercoaster. Last year I had some medication issues and ended up being off work for eight months.
I could not function. At the worst point I was having swings every 3-5 hours.
Even with medication that is working properly. There are times where I go down hill over the course of a few hours. Until I break down and cry uncontrollably for an hour or so. Once I get it out of my system I'm fine.
As others here have said it could also be ADHD. ADHD and BPD can have similar symptoms.

I was diagnosed with it as well. Best wishes to you. But the guy in this question has only a day between each moodswing and even rapid cycling bpd doesn't do that
