Body dysmorphic disorder (what you're describing here, along with symptomatic behavior such as bulemia) is an all-too-common clinical condition. I would really recommend working with a therapist: lack of good nutrition during your teens can bite you big time later in life (I've known women who would otherwise be in great shape athletically, suffering from conditions such as osteoporosis in their 20's).

get advice from a professional nutritionist/trainer. they will tell you exactly what you are and what you can do to become healthy.
Look in the mirror every night and tell yourself you are pretty. Make a list of things that make you beautiful. Try eating a little see that you won't gain weight and announce this out loud. "No matter what I eat I won't get fat" gradually eat bigger amounts while still following the above steps until TADA!!
