Slow down, do some deep breathing, and try to relax when you start to feel highly critical of yourself. No one is perfect and no one needs to be perfect. A minor setback or not doing something right does not take away from who you are as a person. It's only that, a minor setback.
The most important opinion of yourself is your own opinion of yourself! The way that you think about yourself defines how you will respond to every situation in life that you're in. Care less what others' think of you. Wherever you go, there you are. When you realize how little the opinions of others matter, you will begin to put less pressure on yourself to please them.
Common, and not healthy. And it is a vicious cycle. When you are hard on yourself for being less than perfect, and then allow it to prey on your mind, you end up living in a state of chronic stress, which serves only to impair performance even more.
It's quite obvious that you should be kinder to yourself, give yourself a break and so forth. But not helpful without resouces.

There is an exercise called Self Compassionate Letter that seems likely to help, but no reason to stop there!
