If insurance companies are concerned seniors aren't getting benefits from a Medicare Advantage plan, then why isn't the stuff they advertise included?

In original Medicare? I have always found that odd that original Medicare never included dental, vision, or hearing even though all of it is detrimental to health and well being. Why do insurance companies sell supplemental plans instead of asking congress and the government to include it in original Medicare? I mean it was created to expand the lives of seniors wasn't it?
