He needs to work out. I would start with a push-up routine every day and then get into weight lifting. He needs to consume protein throughout every day, but especially on work-out days.

He's not eating enough. If he's eating 4000 calories a day and not gaining weight, he needs to see a doctor.
Tell him to stop being a "gamer" and do P90X or something
