My sister has the same issue with her boyfriend. She actually shared a story with me that I'll share with you. Apparently her boyfriend doesn't like the whole hygenic thing also because he doesn't like feeling cold after a shower (or some crap) and this somehow extends to brushing his teeth. One day she goes to him and shows her teeth and goes "Baby, do I have plaque stuck to my teeth" and he looks at her teeth and says "No". She then says to him "Thank you baby, sometimes I get plaque on my teeth, Just like you have right now, but I usually brush and floss so that it goes away." And thus, she preceded to show him how to brush his teeth properly.
Point being-I'm sure he knew how to properly care for his teeth, but because she brought it up in a direct way while not being accusatory, I guess he felt embarrased and like she would notice if he didn't do it properly. I think you need to be direct but at the same time concious of how to break it to him. B.O and poor hygiene is a turn off- make sure he knows that people notice in a subtle way
