Im skinny and I work in a gym, I do not plan on bulking up but I can tell you how. I just like my runner physique although I am not a runner, but as long as I am fit. 5 months may have lost you 20 pounds or so, but you will have to start up slow. Remember to lift lighter and take your time, it may be obvious, and also remember you will get super sore. Just take it slow the first week and be committed. You may want to drink protein shakes and weight gainers, the cost will be worth it if you want to gain weight. so I'm always one of the skinny guys in the gym but everyone starts somewhere, just make friends with people in the gym be friendly and learn from the big guys

The body has a way explosively increasing muscle when someone is entirely new to weight lifting or "regaining" what was lost when someone takes a long break. I wouldn't be surprised if you notice something after an month depending on the regimen of course. But don't expect to turn into Jay Cutler in 5 months lol.
And 4000 calories? I would double check that because that seems like overkill unless you have an insanely fast metabolism.
Lift light to start or lift your own body weight until you get your strength back. Pull-ups, push-ups etc.

Skinny guy in the gym? I don't think anyone cares to be honest. I think you'll do okay, but I'd recommend you do splits so you get enough resting time.
I'd recommend visiting jasonferruggia.com. He's the best I've found.
if you are self conscious about going to the gym how about buying weights for at home?
