My boyfriend lives in another country. He's sick at the moment so I joked I'll send him an ugly nurse to take care of him. He sent me this pic telling me she's giving him a 30 minute bed bath and not to disturb him during this time. How disrespectful is this?

It all depends on your relationship. If he knows you well enough to know you will take it as a joke, it's fine. If he doesn't and just assumes you will, he took a risk that wasn't very smart. I don't get involved in long distance relationships and your issue is one of the biggest reasons. You can't really get to know a person that way. You need personal contact on a much more regular basis to learn who they really are. If you are not around them in different types of situations, you can't learn the important stuff that comes from regular personal contact.
