Your insurance only covers you and occasional drivers. If your insurance company discovers he's been driving your car on a regular basis, they may deny claims. If he carries insurance on another vehicle, that insurance may cover any damage caused by him.
Is he driving your car for his convenience or for your convenience? If it's for his convenience, ask him what he believes would be fair. He probably won't be receptive to demands. You'd be paying for the insurance whether he was driving or not, and it's not covering him, so that wouldn't be his bill to pay. He is putting wear and tear on the vehicle that wouldn't be there if he wasn't driving it, so how does he plan on compensating you for that wear and tear?

Send him an invoice. If he doesn't pay, go to small claims court
Let him know that since he is driving the car it is legal but not only a good idea to add him to the insurance policy and he should cover the insurance as well. That way you are able to save your money god forbid u have to get a new car if he totals it. Don’t let anyone drive your car honestly. It’s not worth it.
