The first is any sort of cheating. Interactive sexually oriented behavior with a third party. It is the ultimate expression of betrayal. Those other issues can (but not always) be remedied. But these situations are also betrayal and can kill a relationship.

Always takes and never gives. It makes a relationship unevenly yoked.
Lack of respect. It also makes a relationship unevenly yoked.
Always makes mistakes and not learning. This can be worked on and fixed, though.
Lazy or No Ambition. Concern for loss of a relationship can engender ambition.
Refuse to do anything together. Common interest is central to a friendship. Without it, there is none. It is merely exploitation.
Indifferent to a partner's need for intimacy and validation. The real core of a loving relationship.

All of those listed which I would broadly summate to not collaborative except for lazy. Not being family oriented is another as if she is not loyal to her family she won't be loyal to you which is closely related to fidelity. If she seems to like attention from other men and worse flirts it is an indicator she will cheat.
