Both, my mom was born and raised in the Deep South in the 30’s. Every other word out of her mouth was “ nigger” I was also exposed to actual racism through her direct family.
Brainwashing, mostly. Though a single negative experience is usually enough for a person to paint an entire group with a broad brush. It's easier to look at it as "those people" instead of "that person." Simple minds ever have simple views.

I'd guess both potentially but upbringing and environment are likely to be most dominant.
It's ignorance that leads to fear and hate mostly but the things you listed can also play a role.
A lot of young people are taught to hate white people in the leftist schools (and boys).
They meet people in a particular race and find that many are just crap assholes.
I think it can be a little of both, but probably more experience. I didn't hate Asians until I visited Asia and saw how much they hate White people
