Not necessarily. Let's just use the transgender example. I could say I'm a woman to gain certain privileges. That would be pretending. However, the majority of these people aren't pretending. It may look that way to compete in women's sports for instance, but in these cases, in their brain they genuinely believe that they're a woman. They're body is male, but their mind is female, okay? So it's not pretending if they believe it to be true
