My ex-boyfriend bought me a birthday gift that I didn't want or ask for. How do I say thank you?

I've been trying not to speak, email or text him for almost a year. He emailed a short little note on my birthday and I thought that that was it. But then he presented me with this really thoughtful gift that I really didn't want to accept.

I want to say thank you in a way that is faultlessly polite but at the same time lets him know that the present doesn't change anything. I don't want to encourage him, or give him false hope or anything just because he interprets my politeness as something more.

PLEASE HELP! I'm really stuck.
