He made a joke oh then it would come out looking like his mom.. I was like why did u just call me ugly he said cause it was a ugly thing to say so I called him a fat ugly b*tch and threaten to leave him am I overreacting?

Even if she were, that's his mom, you can't assume he's calling you ugly and let alone point it out, unless he's not close to her and doesn't care if you call her ugly. I don't have enough details to know if that's what he truly meant or if he was lashing out at you for something mean you said before that (you mentioned he said you said something mean).
Now, if the insults are escalating that much, sounds like a an unhealthy relationship to me. Respect and love should always be paramount. And if there's no respect, there's not as much love as you may think, from both of you. But if you feel insulted, there's no need to "threat" to break up, you need to be comfortable with your partner and if you are not and there isn't an interest in making things better, just leave.

Well basically I said I would be embarrassed if our baby came out ugly and then I laughed and he said it would come out like his mom then I stormed out insulted him and we had a fight then he said he said a insulting joke bc what I said was offensive and he wouldn’t care if the baby came out ugly or deformed

Well sounds like the one who insulted first was you. He didn't said anything offensive to you, he said the baby would look like his mom. I mean I don't know her, as far as I know she could be the elephan woman, but I don't think her son sees her as ugly and didn't offended YOU, said the baby (half yours half his) would look like her, so sounds like you exploded out of the blue. Maybe you should stop and think if this outburst was the result of maybe a bad day you had and took it out on him or if you actually resent him for something else you hadn't even realised. Either way, doesn't seem healthy to have such pointless fights that come to such harsh insults. You need to learn patience, be understanding and empathic and he should as well or this relationship is turning toxic af in the blink of an eye, you already have one foot on the door to becoming toxic, so beware.
