There's a girl I have mutual feelings for, but one little problem is that I'm 186cm (6'13) and she is 152 (4'98) (And no, she's 21).
I honestly cannot fathom the long-term implications of that, I never dated anyone that short. is it even realistic? I've seen people say difference of 30cm is already getting old after some time but have anyone had ~35?

If height above ground is such a handicap for you, then you don't really love her and you should let her go.
If you had genuine feelings for her, then you would not have anxiety and panic attacks about such a trivial thing. Perhaps that height is what determines your feelings for another person and if that is the case, then I pity you honestly because it makes you a very superficial person that only sees how much profit you can extract from others.
Let her go to find someone that has the decency to love her for whom she is and not for what she does not have.
