Ohhhh, let me explain this one!
Just to clear things up a bit first, once you buy a drink, you have paid for it. If a company says you are welcome to have more of something for free if you buy one, first, you are not stealing, you are not taking advantage, you are going by the policies that they allow, and they allow this because its actually one of the biggest profit makers for restaurants and fast food chains.
Second, soda's are CHEAP. I mean, a restaurant is usually only paying a couple of cents per drink. Unlike canned/bottled products, a restaurant can buy the syrup for the sodas at minimal cost because they are usually contracted to a company who gives them discounts, and the restaurant/chain can make said soda in bulk large batches. A restaurant "can" then water down the drink or proportionately add a lot of ice to a soda giving the illusion a customer is getting a lot further increasing their profits as one is getting less actual soda then in a typical can.
Third, both the fast food chain and restaurant upcharge for your drink. Keep in mind, free refills (here at least) only apply to those that dine in, but those that do drive through are charged the same upcharge as those that can get refills inside even though they can't. Restaurants aren't losing money off of the practice of free refills, they are gaining profit otherwise it would not be a sustainable practice.
The free refills is not about entitlement but getting you in through the door of their establishment so hopefully you will come again or order more. Soda is so cheap. Its like when a bar here gives you free shots, or free nuts or pretzels. It's really all a sales tactic.
Competition is fierce in the US.
Free drink refills (of cheaper drinks) in restaurants, is the result of different restaurant chains trying to convince people who would rather not spend money at a restaurant (or prefer another restaurant) to go to them.
Without free refills, the customer won't order as much food (or any food), because restaraunt food is horribly overpriced compared to making it at home or buying it in a store.
So we actually pay more than is reasonable for eating out, but its advertized in a way that hides that, so it seems cheap.

yes, in most restaurants. In some small diners here, extra refills may cost extra. A lot of places with free refills though, especially fast-food places, it's a way to get you to stay and order more food. And a lot of times it doesn't really cost the restaurant because they are charging more money for food items than they should anyway.
I've been other places and I really do believe some menu items here cost more than they do other places, but it probably depends on the restaurant too.
Yeah, like my movie theater for instance gives you a free on popcorn and soda if you order the jumbo. And since they're already charging much more than they should, they really aren't losing out.
The general rule of thumb for restaurants is too charge 3 times what it costs them to make it

It's not about entitlement. I don't expect free refills when I leave the U. S., because I agree that from a business perspective, it's stupid. But obviously if I had a choice between two restaurants, I'm gonna choose the one that gives free refills. 😂 But apparently some parts of the country don't do this from what I've heard.

It's "common" at fast food joints and some restaurants (usually only for soda, if at all), and that's about it. It has definitely become less common since COVID, probably more for financial reasons than health reasons. I usually drink a lemonade of some sort at restaurants and don't think I've ever gotten a free refill, but they have definitely refilled and charged me for it without asking before.
Water is an exception - you can actually get water for free almost anywhere.

The practice also keeps a customer on the premises longer, so they'll hopefully spend more money before they leave.
When they charge $3 for a 20 glass of ice with a little bit of colored sugar water in it, the margin is so high that the patron can drink a pitcher of it and there would still be a profit. And it gets people to go there and pay for other seriously overpriced "food."
Most places I go to in South Carolina don't have free refills, unless you go to like Chinese restaurants where they'll fill your glass up for free.

In some places there are free refills. It is usually limited to fountain drinks. The establishment charges you 2 bucks for a drink that costs them ten cents because it is mostly ice cubes. Maybe one in ten ask for a refill anyway.
I don't know in USA but in Peru... Where i born... Is or was very common... I'm taking about juice fruit prepare on site but i don't know exactly if that happens the same with food
It depends. Fountain drink mixes are really cheap, so typically money isn’t lost with the initial price of the drink and meal. It’s also something that doesn’t extend once you leave the place, so you can’t return the next day with yesterdays cup.
The concept does not exist in my country. Given the levels of obesity in the United States it might be a wrong idea to begin with.

Free refills is technically not true. Restaurants charge you enough to cover above average an individual could drink within the time at their restaurant.

The idea is to get you to buy other items at inflated prices. Nothing in life is ever free.
