My coworker called me by his ex girlfriend's name?

My coworker who is a part of my office friend group called me over for something work related and by reflex called me by his ex girlfriend name. He has spoken about his past relationships to me and another friend of mine. About how he had a dedicated girlfriend in school, but he couldn't stay with her because he was immature and lead her on.
But he's asked for forgiveness since and they've moved on.
He recently married and he seems to be very happy with his wife.
We have a good working relationship, we only hangout in groups and I think of him as a mature person I can rely on work wise and sometimes for advice too.
But why did that slip out of his mouth though?

He was probably just thinking about her, or something that just happened between them. I know I do that a lot where my mind is in two different places. Like I'll be thinking about my sister and then my roommate with start talking to me and I call my roommate by my sisters name. This might be the case, but I'm not sure
