Gratitude or Crush?

I have a good friend who has some friends who aren't very nice to him and I am one of the few who is nice and kind to him. We have conversations about anything and everything but one day he started complimenting me a lot (he usually compliments me a little) and also calling me things like "cutie" and "sweetie" which he has never done before...

Does he do this out of gratitude or does he have a crush on me?

From gratitude to crush there is a big area, so I think before "crush" you should wonder if he likes you or is attracted to you (which doesn't mean automatically "crush", it can be just light or casual, nothing with expectations/desires behind).
He could totally be starting to like you in "that way". And of course he could totally have a crush as well, but consider also much lighter forms of attraction (they can develop in a crush later anyway, but it's never granted).
Anyway, to understand if he likes you romantically/physically, check something: how does he talk with other female friends? Is he used to call them with cute nicknames like that? If yes then it might be just his "style", if not, then he might be actually trying to communicate you he likes you, in some ways we don't know yet.
The next step is understanding if you like him too, at least a bit, and to maybe bring the conversations with him to this topic, so he can declare what he feels and how actually it is (if just casual attraction or more similar to a crush, or none of that).
Mind that guys, especially young, might be quite slow to even understand themselves, and their feelings. He might have no answers if you ask him right now, so anyway give it some time...

Perhaps it is a bit of both. Maybe he has learned that you are nice to him and over time has developed a crush because of it. Nothing wrong with that. Those things happen when you show gratitude or kindness.

People start to see you for who you are, which is a kind person. The emotions or feelings that follow that can easily turn to romantic interest. The fact that he has so few people that are kind to him tell me that your kindness has struck him.

The question is, do you like him in that sort of way? If not, then don't toy with him or lead him on. If yes? Then reciprocate and even ask him to a casual lunch. You will get a much better understanding of his intentions on a one on one date like activity.
