How do you keep going at the gym?

I gotta admit that I really can't figure out WHY I can't do my best at the gym. I want to be fit, lean, toned and everything but every time I go to do my thing at the gym my patience goes from 100 to a 0 and my motivation suddenly disappearing on me. I really really want to be fit and all that but I can't seem to be at a gym and workout like a normal human being.

does anybody know why this occurs?
and what can I do to make it to go away. I really want this!! And don't even say that I don't want it enough. Yes I do want this badly, but I'm always exhausted, sleepless and I AM eating healthy (with moderation).

please help!
