Congratulations on your decision to get healthy. This IS a lifestyle change and not a short term plan so do keep it in mind.
1. Get an idea of your ideal body weight- To do so you will need to know your frame size. To get an accurate reading, you have to encorporate your frame size, height, age, and gender. So, to get your frame size, take your thumb and index finger and wrap it around your other wrist. If the fingers overlap you have a small frame, if they touch you are a medium frame, and if they do not meet, then you have a large frame. Then, you will want to find an ideal body weight calculator that encorporates this info and then it will calculate a healthy range.
2. Start a food diary- This allows you to have control over your food consumption and makes you become more aware of what you eat. Chances are, you won't eat a slice of pizza or cake if you have to log it and face it directly later on. It also ensures that you are not starving yourself. You want to limit calorie intake but you do not want to starve because that is also unhealthy and counterproducting. My favorite site is www.sparkpeople.com.
3. Eat healthy- eat veggies, fruits, lean meats, whole grains like rice, and 100% whole wheat bread. You can also make low fat hummus which is packed with fiber and nutrients. You can mix and match these however you want and create all kinds of yummy and healthy foods.
4. Exercise!- Now this depends on how active you already are. If you don't feel like you could do a ton of cardio, then I would stick with daily walks until your able to feel totally comfortable walking. Then you gradually want to step it up and walk faster and eventually start running. I also reccommend light weight lifting. You can buy cheap weights anywhere, and do all kinds of workouts. Cardio will help you burn fat momentarily but you stop burning once you are done. By building muscle with weight lifting, you will build muscle that helps burn fat while you do cardio, and all day.
Hopefully I gave you some good enough ideas to get you started :)

To loose weight, you have to get on a healthy diet and exercise. I would suggest joining a gym and getting a personal trainer for couple weeks. They will give you a blueprint of everything you need to do to loose weight design for you.
