How frequently do you switch up your workouts?
I don't want to jump the gun, because I'm sure after two years you know how it goes, but I've personally found that I see the most results in the first week or two after starting a new really challenging workout (and then they increasingly level off). I like to incorporate weights and do what's called "circuit training"--short bursts of cardio between muscle workouts. It keeps your heart rate up the entire time, and muscle burns more calories than fat, so you're burning the max. amount of calories throughout your workout and for the rest of the day. To be honest, I mostly stick to Jillian Michaels workout DVDs. Even the ones for beginners are pretty hardcore in my opinion, and she shows you how to adjust them once you get more advanced.
Anyway, there's that, plus, I've always just heard that you eat small amounts of food all day long and just constantly change what you eat and how you exercise.

You probably need to change you dieting or work outs..thats going to be the not hardest but probably difficult part to get rid of though.
You're body fat is getting really low and you're body doesn't want to let go of any more fat. I would try adding weight training or high intensity cardio to your workout routine.
I'm not positive, but I think your body may have gotten used to the workout if you're doing the same things each time. I suggest altering your workout.

The less weight you have, the harder it is to get off. You'll have to do more rigorous workouts to lose the last few.
